Sickness in Sons of the Forest is a simple cause-and-effect system. Dirty water and spoiled food can make you sick. When it hits, a new debuff icon appears and your maximum health drops hard. That makes caves and raids risky, so prevention matters.
To cure sickness fast, take Medicine, it removes the debuff instantly. You can craft Medicine with Aloe Vera and Yarrow, or loot it from caches near the crash site, campsites, suitcases, and crates.
Do not treat Medicine as a free pass to drink dirty water. It only works while you still have pills. If you are out, you will wish you had kept your diet clean.
Clean water also clears sickness. The easiest passive method is a Rain Catcher. Build it with 12 Sticks and 1 Turtle Shell. Place it near your base and let it fill during rain. Water from a catcher is clean by default.
It needs weather, so I keep two or three placed around my main build to bank extra drinks. If you grab water in a flask out in the wild, boil it first.
If you need water now, use the active method and boil it. Find a Cooking Pot at early camps on the northern beach near the Machete, or at the camp where the Modern Axe spawns.
Equip the pot, collect water from any freshwater source that is not the ocean, then place the pot on a lit campfire. Do not add ingredients if your goal is clean water. Watch for the boil, the lid will bounce, then pop off when it is done. Pick up the pot to carry clean water and drink safely. This is my go-to when I am moving between bases.
If you cannot spare Medicine and you are far from camp, sleeping can also clear sickness.
Often one full sleep removes it. If the debuff is heavy, it can take longer, but sickness does tick down over time even if you do nothing. I only rely on this when I am stuck without supplies.
Prevention is the best cure. Do not drink from rivers or lakes unless you boil it. Do not eat meat that has turned. Dry or cook fresh food, and rotate your stores so nothing spoils at the bottom of your bag. When in doubt, toss it. A safe dinner beats a low HP bar in the next fight.
